There are over 350 different medals and awards available to Service personnel, veterans, their families and civilians. These awards are processed by a number of different departments and organisations. The application process varies according to the medal or award that you are applying for. All forms are available via the application form library and are subject to full assessment by highly skilled assessors.
A number of tools and resources are due to be developed to support you in understanding eligibility criteria prior to making an application. These include the interactive campaign medal map, campaign medal guide, ACSM calculator, LS&GCM calculator, Cadet Forces Medal calculator and digital JPA forms with calculator features to help determine potential eligibility. These tools alone will not guarantee a successful application and cannot be used to confirm eligibility. Tools will be added to the landing page as and when they become available for use.
What you need to do
You are encouraged to check JPA to see whether a medal has already been added before making an application as a number of medals and awards are automated through JPA. Veterans Badges are issued automatically on exit and do not have to be applied for.
If you believe you are eligible for a medal or award, or are unsure, you should speak to your HR Unit Admin in the first instance. All applications should be staffed by unit admin using the correct JPA form.
What you need to do
Civilians deployed on Operations under the Support to Operations (S2O) scheme will have their medals applied for them on their behalf by S2O at their end of tour. Any retrospective applications should be made by the individual themselves directly using HR Form 114.
Those on operations not under the S2O scheme who think they are eligible for a medal should apply using HR Form 114, accompanied by relevant documentation that confirms their eligibility to support their application.
Eligible Civil Servants will be nominated for the Imperial Service Medal on retirement. You should not apply for this award. Further details on eligibility requirements can be found within the Long Service and Efficiency Medals page.

Contractors deployed under CONDO or DCS within DEFCON 697 written into their contract should apply using CONDO Form 5. Contractors deployed under other contracts should contact the MOD Medal Office for further advice on medallic recognition.

There are strict rules around who can apply for a deceased person's medals or medal record. You must be the official next to kin as determined by intestacy law. The medal office is unable to issue medals or medal information to other family members. The application process and form can be found on gov.uk.
