What you need to know
MOD Form 90 is the name for the Military Identity Card and these along with Identity Discs are issued to serving members of the Armed Forces only.
What you need to do
Application for MOD Form 90 and/or Identity Discs are to be submitted via the Unit HR using the tri-service form: F IDENT 1024 (link to form if available).
Multiple applications for ID Cards, with the same reason for issue and which do not require photographs, may be made by the use of a separate nominal role attached to a single F Ident 1034 completed as follows:
- part A - service number box to be annotated with "see attached nominal role"
- part B - replacement reason
- part C - to be applicable to all applicants.
The normal role is to include the following details for each person listed:
- service number
- surname
- initials
- date of birth
- rank
- height
ID Cards may only be replaced for one of the reasons listed on the F IDENT 1024 application form (noting that the rank displayed may be within one rank (up or down) of the holder's current held rank.
Authorisation of application form
All F IDENT 1024's and Nominal Roles must be authorised by the rank Warrant Officer (WO) or above (for Civilian Unit HR Staff this is 'Band D or above') and any instances where paperwork has been returned by the ID Card Section should be checked by the Unit HR if it has the correct authority. Unit HR's are to ensure the forms are correctly completed and signed before sending.

Unit HR are to send completed ID Card / Disk application form F Ident 1024 to the ID Card Section.
Timescales
Demand for routine ID Cards/Discs will normally be completed within 10 working days,
Posting
ID Cards/Discs will be dispatched by 1st Class post where they are non-urgent and in quantities of less than 20. Special delivery, providing a tracking history of the delivery, will only be used for quantities of 20 or more, or for urgent applications.
It is a serious offence for a Service Person to be in possession of a duplicate ID card (unless exceptional permission has been granted by SP Pol).
What you need to do
There are times when ID Card/Discs are required asap, for example if a Service Person is being deployed, and in such circumstances Unit HRs must make an urgent ID card application using the same F IDENT 1024 form (link if available).
Urgent ID Card/Disc requests are submitted in the same way as routine applications using the same F IDENT 1024 form but the form must be annotated with 'URGENT' in the 'Processing Priority' (at the top of the form) and must also be accompanied by an iSupport raised by Unit HR or a Serive Person raised by the EC (at the request of the Unit HR).
Once the iSupport/Service Request has been raised, the reference number fro the iSupport or Service Request must be annotated in the relevant box on the F IDENT 1024 in line with the current ID Card process. An application will not be processed as urgent without the iSupport/SR number appearing on the form.
Authorisation of the application is the same as the routine ID card process.
Unit HR will complete the form and ensure the form is correctly authorised and completed.
Timescales
For postal returns: For UK units, urgent applications received 13:00 Monday-Thursday (13:00 Friday) will be processed and the ID Cards/Discs dispatched, via Royal Mail "Next Day", for delivery by 13:00 the following working day.
For non-UK units, the ID Cards/Discs will be delivered to Northolt sorting office for despatch on the next availale flight.
Collection in person requests - Agents must inform a Supervisor of all collection requests.
Customers can collect ID Card in person from the ID section in Gosport. At the top of the F IDENT 1024 form and in the problem summary of the iSupport/Service Request it must be annotated with 'URGENT' or 'URGENT FOR COLLECTION'.
ISupport/SR's received by 15:30 Monday - Friday may be collected on the same day from the ID Card Section between 08:00 and 16:30. Cards cannot be collected outside of these hours unless exceptional circumstances where psecific arrangements have been made (including weekends and bank holidays).
